There are two forms online for this individual. The form entered above was handwritten. The date of the Medical Examination is partially blurred, it is March 19th, 19??. For previous military experience, this Nursing Sister served from September 28th, 1914 to the date of this form.
The second form is dated March 19th, 1917 and signed in Calgary. Also, on this form her birthdate is given as in March rather than February.
The following appeared in the Calgary Herald on January 30, 1918, pg. 8:
Nursing Sister Phyllis Gilbert, who lately returned from England on sick leave, has resumed her duties at the Military Hospital at Frank.
