Thomas Michael O’Neil was born at Saint-Anicet, Quebec, on 7 August 1895 and worked as a farmer. Drafted under the Military Service Act, he joined the 1st Depot Battalion, 1st Quebec Regiment at Montreal on 16 July 1918 as Private 3087581. His service remained in Canada. On 16 August 1918 he was struck off strength on return to the Registrar's Records at Montreal. Later discharge proceedings confirmed the administrative discharge.

Notes
Medical examination at Montreal on 16 July 1918 recorded age 22 years 11 months, height 5 ft 6¼ in, weight 138 lb, chest 34-38 in, dark complexion, blue eyes and medium-brown hair. Physical development was recorded as good and he was placed in Category C3. A scar on the left thigh, approximately 1½ inches, was noted.

The contemporaneous Casualty Form records him struck off strength on return to the Registrar's Records effective 16 August 1918 at Montreal. A later Short Form Proceedings on Discharge records the same effective discharge date and reason and confirms the discharge at Montreal on 6 March 1919. The file contains no evidence of overseas service, medical discharge, court-martial discharge, resignation or death in service.
